Chronic Care/Principal Care Management
Our goal is to identify your most at risk patients first, those suffering with chronic conditions as they will benefit most from the Advanced Healthcare Management Programs (AHMP) we offer. Using the list of over 19 chronic conditions recognized by the Centers for Medicare and Medicaid Services (CMS), patients with 1 chronic condition are placed in the Principal Care Management (PCM) group and those with 2 or more fall into the CCM group. The focus of both CCM and PCM is on providing structured care anPrincipal Care Management (PCM)d coordination to improve health outcomes.
Remote Physiological Monitoring (RPM)
We utilize wireless (4G) medical devices such as weight scales, blood pressure cuffs, glucometers, and pulse oximeters to capture and transmit a patient’s vital signs, symptoms, and other health-related information directly to healthcare providers.
If a patient is identified to be a part of the Principal or Chronic Care Management program, they also are a part of the Remote Physiological and/or Therapeutic Management program.
Remote Therapeutic Monitoring (RTM)
RTM is a Medicare-defined service that allows healthcare providers to remotely monitor and manage a patient’s adherence and response to therapy – whether Physical Therapy (PT) or adherence to a medication plan.
Unlike Remote Physiological Monitoring (RPM), which focuses on vital signs like blood pressure or glucose, RTM tracks:
- response to treatment or medication plans over time,
- patient symptoms (e.g. pain levels, medication side effects),
- therapy adherence (e.g. usage of an inhaler or physical therapy device).
Data may originate from entries the patient records on a PC or mobile app or it may be the result of a transmission from a smart device (often connected via 4G, Wi-Fi, or Bluetooth). This is one of the significant differences between RPM and RTM reporting and the resultant billing. Billing for patients using RPM must be the result of automated recording and transmission of data. While for RTM, certain types of data can be recorded and sent manually by the patient or caregiver.

Step One: Assessment
Begin with a free, no obligation, comprehensive evaluation of your current patient files. We use a fully automated, in-house proprietary process to generate a Prioritized Patient List consisting of your most at risk patients.

Step Two: Implementation
Once your staff has approved the Prioritized Patient List, our Onboarding Team will begin contacting the patients to verify their willingness to participate. We then begin shipping the necessary products to each patient and scheduling individual or group training sessions as necessary.

Step Three: Billing
Our system tracks each contact made with a patient and reports those contacts as billable time against the appropriate CPT code. This also provides an extensive audit of each transaction. At the end of the month, we will import the billing hours into your EHR for transmission to the insurance provider.
